Sirona Recovery partners with the Milwaukee County Housing Division to provide support to chronically homeless individuals with a disabling condition. The goal of CHCM is to move people from living on the street or in a shelter into self-selected housing as quickly as possible without preconditions of treatment or sobriety.
SHCMS is a collaborative process in which the participant and case manager work together to formulate a plan including goals that address the participants wants and needs while removing barriers to securing and maintaining safe and stable housing. The case manager works closely with area landlords and housing programs when assisting participants to find housing and advocates for participants once housed. Case managers also help participants make connections to community services, offer recovery-oriented services, help with social security and disability applications, obtain job training, and complete applications for job, volunteer, or community services positions. Chronically homeless families and individuals are referred to the program through the Coordinated Entry Process managed by Impact 211.
