Case Manager

The Deflection/LINK Program provides a public health–centered alternative to arrest or prosecution for individuals experiencing substance use, mental health, or social challenges. In partnership with the Ozaukee County Sheriff’s Office, this program helps participants access treatment, recovery support, and community resources before or instead of deeper involvement in the justice system. The case manager plays a critical role in bridging the gap between criminal justice and behavioral health services by providing direct case management, linkage, and follow-up support to participants referred through various pathways, including self-referral, first responder, law enforcement, and jail re-entry. The case manager will conduct assessments, develop individualized case plans, provide ongoing support and accountability, and coordinate communication between participants, service providers, and law enforcement partners. This position requires a balance of clinical insight, system navigation, and relationship-building to promote recovery, reduce recidivism, and support positive community reintegration.

Please note: This position will see clients in the community and in the office.

Responsibilities:

  • Respond promptly to referrals and ensure timely participant engagement, including rapid completion of intake assessments and enrollment steps to minimize delays in service initiation.
  • Conduct comprehensive intake and screening assessment using evidence-based tools (e.g., ASSIST, PHQ-9, GAD-7, C-SSRS, Social Needs/SDOH).
  • Establish rapport with participants through a trauma-informed and person-centered approach.
  • Assess needs related to substance use, mental health, housing, employment, social support, and other social determinants of health.
  • Develop individualized, strengths-based case plans that support recovery, stability, and personal goals.
  • Coordinate linkage to community resources, treatment providers, and recovery supports.
  • Identify and remove barriers to engagement (e.g., transportation, documentation, scheduling).
  • Maintain regular check-ins and progress reviews.
  • Collaborate with the Ozaukee County Sheriff’s Office, treatment providers, and other partners to support participant progress and program completion.
  • Complete all documentation accurately and promptly to meet program, grant, and agency compliance requirements.
  • Maintain accurate participant tracking and reporting, including timely updates to referral sources such as law enforcement when a referred participant enrolls, completes, or is discharged from the program while maintaining confidentiality and ethical standards.
  • Participate in multidisciplinary team meetings and case consultations.
  • Represent the LINK Program in community and interagency meetings.
  • Support data collection and reporting efforts to meet grant and program outcomes.

Qualifications and Competencies:

  • Bachelor’s Degree in Social Work or a related field; Master’s Degree preferred
  • At least one (1) year of experience working in the Mental Health &/or Substance Abuse field preferred
  • High level of skill in organization, attention to detail, accuracy, and the ability to handle multiple tasks at one time
  • Clinical knowledge and skills with the ability to clearly communicate these to others
  • Experience with Trauma-Informed Care and harm reduction approaches
  • Ability to maintain and promote positive working relationships with others
  • Excellent assessment and interviewing skills as well exercise good judgment
  • Ability to work well with individuals from diverse economic, racial, and ethnic backgrounds and LGBTQ+ identities
  • Pass a pre-employment background check
  • Valid driver’s license and automobile with insurance
  • Working knowledge of Microsoft Office (including Outlook, Word, and Excel) & web-enabled databases
